Our services include:
Home Health Care Services (Medicare Certified): For those needing treatment or recovery at home after an illness or injury, we provide skilled nursing and high-tech services, physical, occupational and speech therapies, medical social worker services, home health aide/homemaker services and coordination of home medical equipment needs. These services are certified under Medicare, Medicaid and private insurance.
Specialty Clinical Care Programs: Patients with specific diagnoses, conditions or rehabilitation needs may benefit from our specialty clinical care and disease management programs. These programs are appropriate for patients who need help managing their diabetes, heart condition, mental illness, Alzheimer’s, or non-healing wound or those who are recovering from joint replacement surgery.
Private Duty and Personal Care Services: Whether in a private residence or an independent living facility, we can provide everything from round-the-clock highly skilled medical attention to a weekly visit for help with personal care and the activities of daily living.
Catastrophic Care Solutions: This in-home care program responds to the needs of patients requiring continuous care following a catastrophic brain, spinal cord, automobile accident or work-place injury. Post-surgical patients or others who are chronically disabled may also benefit.
Community Care Programs: These services are provided under contract with state and local assistance programs in each of our locations. We provide home and community support programs for low-income elderly and disabled clients.
Health Screenings and Wellness Programs: These are provided to groups at no cost in areas where we have existing locations. Topics include fall prevention, diabetes control, managing the challenges of aging, and preventing infections.
Senior Communities: We offer a full range of services, from personal care and supportive services to health care management.
