Our Medicare Certified Home Health Care Services are provided by nurses, therapists, social workers, and home health aides. Our team are experts at providing personable, supportive care for people recovering after an illness or injury, who are disabled, or who need assistance to remain at home because of age or illness. We can provide everything from round-the-clock highly skilled medical attention to a weekly visit for help with personal care and the activities of daily living. Our private duty and personal care services respond to each person’s unique needs, ranging from personal care and hygiene and homemaker assistance to transportation and companionship.
Catastrophic Care
THE MEDICAL TEAM’s catastrophic care services are for those who need continuous care after a life-changing injury or chronic disease, with specialized expertise in caring for spinal cord and traumatic brain injuries. These services, available exclusively in Michigan at present, work to ensure the best possible outcome for patients after transitioning from the hospital into their home or a rehab facility, from assisting with discharge to developing a plan to serve custom patient needs. These services may include,

Hospice Care
As life’s journey nears its end, THE MEDICAL TEAM is dedicated to supporting patients and their loved ones. We are committed to providing care with compassion, skill and respect.
We provide comprehensive hospice care to support the patient’s physical, emotional, and spiritual needs, including medication related to terminal illness, supplies, and equipment. Every patient’s team includes a nurse, social worker, chaplain, home health aide, music therapist, and trained volunteers who can provide extra services such as friendly visits and light household chores.
