About Our Specialized Memory Care Services

Our dementia and Alzheimer’s program provides caregivers of persons with Alzheimer’s disease and related dementias with training on how to understand and cope with the many difficulties they face, such as resistance to bathing and dressing, wandering, and agitation. Emphasis is placed on ensuring high quality of life standards for the patient.

Dementia & Alzheimer’s Program Components:

Patient’s health, functional status, and home environment assessment

Development of an individualized care plan

Assistance with personal care and the activities of daily living (dressing, feeding, ambulation, etc.) provided by specially trained home health aides

Respite care to relieve family caregivers

Informative handbook for family caregivers on problems and coping strategies

Coordination and communication with the patient’s physician and other health care providers

Outcomes tracking of key measures of quality of life and caregiver satisfaction

Program Staff

This program is staffed by home health aides who receive ongoing and thorough orientation and training, including 10 hours of intensive training on the behavioral aspects of Alzheimer’s disease.
